What To Expect When You Stay With Us
- A comprehensive initial assessment is completed upon admission. Therapists will discuss your level of function before admission, evaluate your current status with a detailed assessment and answer any questions about what to expect within the therapy program.
- A plan of care will be established centered on your personal goals and functional abilities.
- Therapy is offered up to seven days a week, based on your individual needs and condition. Therapy sessions will focus on your specific needs for recovery to maximize your improvement for a safe and successful return home. Each treatment provided in therapy is developed to be an individualized session that’s right for you. Therapists will focus on a variety of skill elements that may include balance training, fall prevention, pain management, safe walking and mobility, exercise, training and techniques for improved independence in dressing, bathing, cognition, swallowing and communication.
- Therapists work with an interdisciplinary team, including your physician, to monitor progress and make recommendations to maximize your recovery.
- We recommend wearing loose-fitting clothing that allows movement and proper footwear during therapy sessions.
- In-home therapy assessments are available when appropriate.

This program affords you the option of having a team member as your home care therapist when discharged. Our in-home individualized treatment plan is one of the best ways to get you comfortable being back home while you continue to gain strength and confidence with your daily activities.

Many people would like to continue to build their strength and confidence for the months and years to come. Return to one of our skilled nursing facilities to receive outpatient therapy giving you a familiar location to continue your recovery with your favorite therapists.
We also offers rehabilitation services to new patients through our Medicare-certified outpatient rehabilitation agency locations or in the patient’s home.
